About the Examination
The Immigration Medical Exam, commonly referred to as an Immigration Physical or I-693 exam and can only be conducted by a designated Civil Surgeon. The U.S. Citizenship and Immigration Services (USCIS) grants permission to qualified physicians (M.D. or D.O.) to conduct these examinations. Dr. Bret Haymore M.D., is acting Medical Director at Passport Health Oklahoma and is a designated Civil Surgeon by USCIS.
Form I-693 is used to report examination results. A physical and mental health screening is completed in addition to review of vaccination records and in most cases a TB skin test and blood draw. Depending on the applicant’s personal medical history and other health factors, additional screening or testing may be required. Following the first visit, the applicant will be seen in clinic for a second follow-up visit typically 48 hours later.
At Passport Health Oklahoma we stock all required vaccinations in clinic which can be given on the first or second visit. Once the I-693 is completed it will be given to the applicant in a sealed envelope for delivery to USCIS. A second copy in an unsealed envelope will be provided for personal records.
