Follow Bell Road east to 79th Avenue. Turn left and follow 79th until you reach Arrowhead Summit Apartments. Passport Health will be in the building complex to your right.
From ASU West Campus:
Take 51st Avenue north to Bell Road. Head east on Bell Road and turn left onto 75th Avenue. Turn right onto St. John Road the right onto 79th Avenue and then left into the building complex when you are across from Arrowhead Summit Apartments.
