Please complete this prior to your appointment. If you are unable to access the Intake form, please complete the Pre-Visit Questionnaire. Services offered in the Dallas-Fort Worth area and throughout TEXAS:
We provide in-office travel vaccinations and travel advice in the Dallas-Fort Worth area. And we also will do on-site vaccinations for civilian and military personnel throughout Texas where there is not a local Passport Health office. We use local licensed nurses who have been trained by our regular office nurses or we travel to your location with all needed supplies and vaccines. We are one of the largest independent vaccine providers in the United States, seeing close to 50,000 patients a year. We do provide on-site routine immunizations and flu clinics at many large school districts such as the Dallas and Fort Worth Independent School Districts. We regularly travel to many Texas cities such as Amarillo, Texarkana, Lubbock, Midland, El Paso, Waco, Victoria and many others. We also provide Wellness Screenings including anonymous results summaries as follows: Finger prick Cholesterol/Blood Sugar, BMI, Blood Pressure and all Blood Draw and Urine Draw lab tests such as PSA, Blood Cell Counts.
