Under civil surgeon supervision, your immigration physical at Passport Health includes a complete physical examination, TB blood testing, syphilis and gonorrhea screening (ages 15+), a mental and physical health review, a substance use assessment, and a full vaccination audit. Required immunizations — including MMR, polio, Tdap, hepatitis B, and others — are all available on-site.
Be sure to bring the follow for your immigration medical examination:
- Personal vaccination records (in any format or language)
- Photo ID for adults; passport or birth certificate for children
- TB treatment records or prior chest X-rays, if applicable
- Completed Part 1 of Form I-693 (parents complete this for minors)
Your sealed I-693 is typically ready within 10 to 14 days of your first visit. You’ll receive a sealed envelope for USCIS submission and an unsealed personal copy. Payment is due at each point of service; cash and major credit cards accepted.
Pricing can vary based on a variety of factors including which tests are needed and if you need to receive any vaccinations. Passport Health does provide an itemized invoice in case you’d like to submit any vaccinations to insurance for reimbursement.
