The following locations provide immigration physical examinations in Northern New Jersey. All physicals are conducted by a USCIS-designated civil surgeon.

During your visit, a Registered Nurse will:
- Review your medical history in detail
- Begin the I-693 documentation process
- Assess your vaccination records and administer any outstanding immunizations including MMR, Tdap, hepatitis A, hepatitis B, varicella, meningococcal, and others
Once lab results are in, you return for your formal physical exam with a USCIS-designated civil surgeon. This visit covers a comprehensive hands-on physical, a mental health and substance use screening, review of all lab results, and completion and certification of all remaining I-693 sections. Your sealed document is then prepared for USCIS submission.
Passport Health recommends doing the following for your appointment:
- Vaccination documentation
- Government-issued photo ID
- Prior TB treatment records, if applicable
- Completed Part 1 of Form I-693
Provided there are no medical complications or follow-up requirements, patients typically receive their completed I-693 documents — in a sealed envelope ready for USCIS submission — within 10 to 14 days of their first appointment.
