Under the supervision of our USCIS-designated civil surgeon, your immigration physical at Passport Health includes a complete physical examination, TB blood testing, syphilis and gonorrhea screening (ages 15+), a mental and physical health review per DSM and WHO standards, a substance use assessment, and a full vaccination audit. Required immunizations — including MMR, polio, Tdap, hepatitis B, and others — are all available on-site.
Be sure to bring the following items to your immigration exam:
- Personal vaccination records (in any format or language)
- Photo ID for adults; passport or birth certificate for children
- TB treatment records or prior chest X-rays, if applicable
- Completed Part 1 of Form I-693 (parents complete this for minors)
Upon completion, you receive a sealed I-693 envelope for USCIS submission and an unsealed personal copy. The civil surgeon’s signature is valid for 60 days from the date of signing. USCIS must issue a decision within two years of your I-693 submission. All fees are due at time of service; we accept cash and major credit cards.
