• Completed or ongoing treatment of noted diseases
• Documented vaccinations
• Mumps, measles, rubella, polio, tetanus, diphtheria, pertussis, hepatitis A & B, Hib, rotavirus, meningitis, varicella, flu (in flu season), pneumonia and COVID-19 (with some exceptions)
