Our numbers are 817-756-6562 and 469-241-1954 or e-mail us at info@PassportHealthTexas.com.    After you make an appointment,  you should receive an Intake form in your e-mail.   Please complete this prior to your appointment.  If you are unable to access the Intake form,  please complete the Pre-Visit Questionnaire.   If you have an upcoming trip, or you are considering having someone come on-site for a vaccination clinic,   call us with any questions.   We offer competitive vaccine prices and superior service.

Services offered in the Dallas-Fort Worth area and throughout TEXAS:   

We provide in-office travel vaccinations and travel advice in the Dallas-Fort Worth area.  And we also will do on-site vaccinations for civilian and military personnel throughout Texas where there is not a local Passport Health office.  We use local licensed nurses who have been trained by our regular office nurses or we travel to your location with all needed supplies and vaccines.   We are one of the largest independent vaccine providers in the United States,  seeing close to  50,000 patients a year.  We do provide on-site routine immunizations and flu clinics at many large school districts such as the Dallas and Fort Worth Independent School Districts.  We have very professional nurses who are carefully trained and tested to ensure every vaccine is administered properly.  We also provide on-site immunization clinics for military deployment personnel and are able to conduct all required screening tests utilizing  portable equipment.      We regularly travel to many Texas cities such as Amarillo,  Texarkana, Lubbock, Midland, El Paso, Waco, Victoria and many others.  We also provide Wellness Screenings including anonymous results summaries as follows:  Finger prick Cholesterol/Blood Sugar, BMI, Blood Pressure  and all Blood Draw and Urine Draw lab tests such as PSA, Blood Cell Counts.

International Travel Documents Proper Immunization Documents: With us you’ll receive an official immunization record to keep with your international travel documents, along with a customized travel health and travel information booklet.   The International Certificate of Vaccination (shown left) is required for entry into many countries.You will also receive a travel health booklet  that details the health risks and travel vaccine requirements for the specific countries on your itinerary, plus consular information, general travel advice and tips on avoiding travel health issues such as travelers’ diarrhea and other maladies. With Passport Health, you will receive one-on-one counseling from a qualified travel health nurse.

 

International Travelers should verify their history of vaccination against diphtheria, tetanus, pertussis, measles, mumps, rubella, varicella, poliomyelitis, hepatitis B, Haemophilus influenzae type b, and pneumococcal invasive disease before traveling.  If there are gaps, immunization should be obtained, whether or not international travel is planned.    As one of the largest private vaccine purchasers, Passport Health provides affordable travel vaccines that doctor offices can not provide.
